On this day of 1878 before me, George C. Phelps, Judge of the Probate Court, within and for the county and state aforesaid, James W. McCracken who being duly sworn makes oath and says that he is a practicing physician of 12 years standing and that on or about the first day of Oct. 1866 he was called upon by Samuel Shroyer for medical treatment & that he found he was suffering from intense pain in the bowels, that he has been suffering from an attack of what seemed to be ar__ey diarrhea, and followed by obstinate constipation and congestion of the mucous coats of the intestines ____ tenderness over the descending colon and about ______ flexure- fistula in ano developed and all evidence of long continued d____ diarrhea. This condition has continued until the present time. The severe attacks of pain more frequent. J. W. McCracken.
