The State of Ohio Wood County ss
On this 29 day of August A.D. 1879, personally appeared before me, W. S. Eberly Clerk of the Court of Common Pleas, within & for the county & state aforesaid James Long and Samuel Werner, who being duly sworn makes oath and says that they were well and personally acquainted with Samuel Shroyer in 1865 and that to their own personal knowledge of the facts in the case they are able to testify & do hereby testify that said Samuel Shroyer was greatly afflicted with chronic diarrhea and fistula in ano when he returned home from service in 1865 & that said diseases have continued from that time to the present. That this affidavit is made from their own personal acquaintance with said Shroyer & from their own personal knowledge of the facts in the case & that they are not interested in the prosecution of this claim.
