The State of Ohio Wood County ss
On this 30th day of September A.D. 1879, personally appeared before me, W. S. Eberly, Clerk of the Court of Common Please within & for the county & state aforesaid Warren L. Chadwick & John W. Brownsberger who being sworn according to law, makes oath & says, that there were privates in the same company with Samuel Shroyer and were taken prisoners at the same time he was & was confined at Belle Island with him. That while there confined in prison said Shroyer was taken with fistula in ano. That said disease was contracted in prison & that they said affiants make this affidavit from their own personal knowledge of the facts in the case & are not interested in this claim & their post office address is Bowling Green Wood Co. Ohio.
